To report an injury, accident, or safety concern, please click on the Origami Portal icon. All incidents must be reported within 24 hours of occurrence or discovery. These incidents include employees, students, staff and third-party members. If you experience any issues accessing the portal, please contact [email protected] at East Texas A&M.

• Select Origami Portal.
• Select from the available options listed to report or submit incident, concern, or application.
• Once you have determined the type of report or application, please follow instructions provided.
• Once completed, click the Complete Incident tab.
• Submit
