All Incidents involving employees, students, staff, and 3rd party members must submit reports within 24 hours of occurrence or discovery. To report an injury, accident, or safety concern, please click on the Origami Portal icon below. • Select from the available options listed to report or submit incident, concern, or application
• Once you have determined the type of report or application, please follow instructions provided
• Once Completed click the Complete Incident tab
• Submit
