Waiver, Indemnification, and Medical Treatment Authorization

Individuals who are participants in events/camps should sign this waiver.

Risk Assessment Form

For any event that takes place on campus, the Risk Assessment form must be completed 2 weeks in advance of the date of the event.

Transportation Request Form

Individuals who are in need of a vehicle for transportation should complete this form.

Event Travel Risk Assessment

For any travels that need to take place, please confirm the Event Travel Risk Assessment.
